ÐÓ°É´«Ã½ Foundation Check Request Form

Use the ÐÓ°É´«Ã½ Foundation Check Request Form when you would like the ÐÓ°É´«Ã½ Foundation to print and mail a check from your foundation account. You must provide back-up documentation and your dean\department chair needs to sign the Check Request Form.

Check requests received before 5:00 p.m. on Fridays will be printed and mailed the following week.

ÐÓ°É´«Ã½ Foundation Deposits

Use ÐÓ°É´«Ã½ Foundation Deposit Form when making a deposit to a Foundation Program Account. Fill out and print the form and send the associated checks to the ÐÓ°É´«Ã½ Foundation Office for deposit as soon as possible.  We want to thank donors promptly for their donations.

ÐÓ°É´«Ã½ Foundation Cash Deposits

The ÐÓ°É´«Ã½ Foundation accepts no cash. Bring cash to the ÐÓ°É´«Ã½ Cashier's office along with a completed copy of the ÐÓ°É´«Ã½ Foundation Cash Deposit form.

HOSTS - Help Our Students To Succeed: Provides financial assistance to students that face unexpected challenges, and helps keep at-risk students on campus and completing their degrees.  Faculty and Staff can refer a student by using the . (requires district login)

Payroll Deduction Form

The ÐÓ°É´«Ã½ Foundation Payroll Deduction Form can be found on InSite in the HR-Payroll tile and is for faculty and staff – full time and part time – who would like to set up automatic, payroll, tax deductible donations to support the ÐÓ°É´«Ã½ Foundation. You can designate where your funds will be used such as an  ÐÓ°É´«Ã½ Foundation Scholarship, Program Account or Wherever the Need is Greatest  and the dollar amount ($1.00 minimum) to be deducted from each paycheck. You can set up contributions of any amount to a single or multiple scholarship or program accounts.
